Diabetes Education - Southport and Formby

The Lancashire Care diabetes team provide group education sessions for people with both newly diagnosed and established diabetes. We call the sessions Diabetes and You and Diabetes and More for those with Type 2 diabetes . We run The 4 Steps Programme for those with Type 1 diabetes. They are for patients registered with a Southport and Formby GP. 

Diabetes Education and Self Management for Ongoing and Newly Diagnosed (DESMOND) Central Lancashire

Diabetes Education and Self Management for Ongoing and Newly Diagnosed (DESMOND) is a national Diabetes structured education programme for people with Type 2 diabetes offers:

  • A programme for people newly diagnosed with Type 2 Diabetes
  • A Foundation programme for people with Type 2 diabetes who have had their Diabetes for more than a year
  • A BME programme for people with Type 2 Diabetes from the South Asian community in English, Urdu or Gujarati (available Preston,Chorley and South Ribble)

Diabetes Specialist Service

The Specialist Diabetes Service supports people within the Preston, Chorley and South Ribble area who require specialist support in order to look after their Diabetes. The team consists of specialist nurses, dietitians, Link Worker and administration support staff who work with individuals, their families and carers when specialist care is needed.
