Community Matrons

The Community Matron Service provides care in the community to meet the health needs of individuals/families within Blackburn with Darwen.

Community Nursing Single Point of Access

The Single Point of Access (SPoA) is provided for the Community Matron Service and District Nursing Service in Blackburn with Darwen, which will triage referrals to the relevant intervention as part of the stepped care model.

District Nursing

The District Nursing Service is available in Blackburn with Darwen, East Lancashire and Central Lancashire.

Nurse Led Ear Care Service

The aim of the Blackburn with Darwen Nurse Led Ear Care Service is to provide an easily accessible and responsive service for a variety of ear problems.

Out of Hours Community Nursing Service

The aim of the Out of Hours (OOHs) Community Nursing Service is to provide an easily accessible responsive service, which meets the identified nursing needs of individuals and their carers either within their own environment or at the Primary Care Unit.

School Nursing

Our service works with children and young people aged 0-19 years and their families or carers. We sometimes work with families before the birth of a new baby.

Special Needs School Nursing Service

The special needs school nursing service exists to provide skilled evidence based care to all special needs children and their families in the special school settings,  providing support for the parents, carers and school staff in meeting the health needs of children with complex needs 

Tissue Viability and Vascular Service - Blackburn with Darwen

The Tissue Viability and Vascular Service is based in Blackburn with Darwen, and may visit you for a range of different reasons.
