Summary

The Lancashire Care diabetes team provide group education sessions for people with both newly diagnosed and established diabetes. We call the sessions Diabetes and You and Diabetes and More for those with Type 2 diabetes . We run The 4 Steps Programme for those with Type 1 diabetes. They are for patients registered with a Southport and Formby GP. 

What we do

We are a team of registered health professionals that have come together to support people with diabetes to understand and manage their condition with more confidence. We are passionate about dispelling the myths surrounding diabetes and helping people to look after themselves.

Our specialist team includes nurses, dietitians and podiatrists that will provide you with the most up to date evidence based information about diabetes.

We collaborate on the diabetes education sessions, joint dietetic/nurse clinics and high risk foot clinics.

How to access the service

You can be referred to the Diabetes Education programmes in the following ways:

  • Be referred by your GP or practice nurse
  • Complete the self-referral form below – once you have completed the self referral form it can be posted or emailed to our team
